IMPORTANT REGISTRATION INFORMATION
Please read carefully before completing your registration and contact the organisers if you have any questions.

Online registration is only available with payment by Credit Card - Mastercard, Visa and American Express only. If you wish to pay by some other method please download a registration form below which contains alternative payment options.

Registration Process
1. Read the registration information and options carefully and make your selections.
2. Complete your personal details.
3. Proceed to the second page to make your registration and program selections.
4. Select your registration payment type – Full conference registration OR conference plus workshops OR workshops only.
6. Select any Social add-ons - Meet & Greet and Gala Dinner tickets.
7. Select your optional sessions - pre-conference workshops, breakfast and concurrent sessions.
8. Select your payment method and submit the form.

Cancellation Policy
All cancellations must be advised in writing.
Before Monday 1 April 2013 - A full refund less a $50 cancellation fee is available.
After Monday 1 April 2013 - No refunds will be given after this date due to fixed cost commitments at the venue. APNA can not accept responsibility for changes to work commitments or personal circumstances. Registrants must take into consideration that venue, food and beverage, learning materials, presenters, etc are finalised and confirmed prior to the commencement of the conference and therefore considerable and non-refundable costs would have been incurred by APNA. A substitute can be nominated to take your place with no additional cost or penalty.

- Complete a separate registration form for each delegate. All fees are inclusive of 10% GST.

All registrations to the conference include:
- Conference morning/afternoon teas and lunches on days of attendance
- Access to sponsored breakfast sessions (bookings are essential and includes complimentry breakfast)
- One ticket to the Meet & Greet function on Thursday evening

A series of optional (full and two hour) workshops will take place on Thursday 2 May from 9:00am – 5:30pm. The conference will then officially commence with the ‘Meet & Greet’ function on Thursday 2 May and conference sessions will commence on the morning of Friday 3 May.

PRE-CONFERENCE WORKSHOP CHOICES

All Pre-Conference Workshops (PCW) are additional registrations. Please pay for them separately and select which workshop(s) you wish to attend where indicated on the registration form. Tickets will be issued at the Registration Desk when you collect your name badge on arrival at the venue.

**IMPORTANT NOTE: As the AML Alliance Leadership in Action workshop and the Flinders University Coordinated Veterans Care: Care Planning and Coordination using the Flinders Program™ workshop are fully funded through government grants; there is no additional charge to participants. 


CONFERENCE CONCURRENT SESSION CHOICES
If you have selected a One Day Registration or Conference Only registration types, please select which Concurrent sessions you would prefer to attend on Friday 3 and/or Saturday 4 May.

BREAKFAST SESSIONS (Included in full Conference registration only)
You can select up to two breakfast sessions if you wish to attend – one for Friday and one for Saturday. This is included in your conference registration.

SESSIONS
Selecting your conference sessions.
There are quite a few choices and it is advisable to take a few minutes to read the workshop choices carefully before making your selections. Please note the different timings of sessions to ensure you do not select sessions that are on at the same time. These include:

Pre-Conference workshops (PCW's) - These can only be selected if you have selected a workshop registration.

Friday & Saturday - concurrent sessions. To assist with catering and room allocations, please indicate which sessions you are likely to attend. These selections can be changed at any time by contacting the organisers.
