National Association of Diabetes Centre: Promoting Excellence in diabetes care

The National Association of Diabetes Centres (NADC) supports all healthcare services to deliver better health outcomes for people with, or at risk of, diabetes. To achieve this goal, NADC provide member organisations with resources that facilitate and promote improved standards of diabetes care. The NADC supports primary care services that are time poor but need diabetes resources at their fingertips. NADC membership for primary care services is only $65 per year which provides access to all staff of that service to access the library of tools and resources.

Teen Clinic is a nurse-led clinic providing teenagers with the opportunity to talk to a nurse about issues such as STI screening, contraception advice, relationship concerns, mental health issues, homelessness or general health advice and education.  

The nurses work at the top of their scope, advising young people to make educated, healthy choices. If their problem falls outside of the nurse's area of expertise the teenagers may be referred on to a GP or to other local service providers. 

Teen Clinic is set up within the general practice environment. No appointment is necessary, and all visits are free.

End of Life Directions for Aged Care (ELDAC) is a project funded by the Australian Government Department of Health to support quality care for older Australians at the end of life.

Five evidence-based toolkits bring together the latest tools, information and resources for workers across five areas: home care, primary care, residential aged care, legal issues and working with other providers.

The ELDAC toolkits are free and available 24/7 on the ELDAC website, with a supporting Helpline (1800 870 155) during office hours.
