Transition to Practice Communications Toolkit

The Australian Primary Health Care Nurses Association (APNA) needs your support in helping us reach nurses working in primary health care, informing them about the benefits of participating in the Transition to Practice Program or TPP. 

The toolkit is designed to provide you with suggested content to share in your newsletter, on your website or on social media channels. You can share it with your communications team or ask us to brief them on your behalf.

Key messages

Key messages are the main points and facts about the NiPHC Transition to Practice Program. They are short statements to help people understand. These messages can be adapted for use across a range of communications.

  • The NiPHC Transition to Practice Program provides an evidence-based education framework over ten months to nurses who have transitioned to primary health care (PHC) and feel they would benefit from additional support.  
  • Fully funded places are available to transitioning nurses under the Nursing in Primary Health Care Program (NiPHC), an Australian Government initiative.   
  • The program runs for 10 months including eight months of clinical and professional mentoring, with additional support provided by APNA for up to 12 months. 
  • The program is looking to recruit both participants (mentees) and experienced PHC nurses to guide them (mentors.)  
  • There is no cost for transitioning nurses to take part in NiPHC TPP.   
  • Mentors are paid $2700 to provide a minimum of 40 hours of clinical and professional mentoring (directly and/or indirectly) over eight months to a maximum of three nurses. 
  • There are two intakes of TPP planned for 2024: March and July.  
  • Supporting employment pathways for graduate nurses who do not yet have a job in PHC. Graduate nurses who are actively seeking employment in a PHC workplace are encouraged to:
