Intermediate NiPHC CDMHA

Nearly 95% percent of the population have at least one chronic condition, and nearly 80% have two or more. APNA's 6-month program offers an intermediate level of Chronic Disease education, designed for nurses by nurses in primary health care to increase their scope of practice in specific health conditions.

General program information

Duration: 6 months  

Time commitment:

  • Compulsory:  5 x 2 hour virtual workshops (Up to 10 CPD hours)
  • Recommended: Community of Practice where resources will be shared

Location: Online workshop. Remaining activities are online.

Cost: $TBA (ex GST) for APNA non-members and members. Grants and sponsorship opportunities are available. 

Eligibility: Registered Nurses, Enrolled Nurses, Midwives & Nurse Practitioners.

Program components

Workshop

An online workshop, split into five parts with one workshop per month, nurses will be guided through their role in empowering patients to manage chronic disease and healthy ageing. The Key focus areas are:

  1. Cardiovascular Health
  2. Cancer control
  3. Injury prevention and control
  4. Mental Health
  5. Diabetes Mellitus

Upon registering for the workshop, nurses will complete a self-assessment (online survey) to identify their current knowledge and identify any areas for improvement.

Community of Practice

A private messaging platform where program participants can discuss, engage and consult with their peers. APNA provides tips and resources to the group, as well as encouraging and facilitating discussion.

Follow-up support

This will include one complimentary call for non-members and ongoing support to members, through the APNA Nurse Support Line – a service to assist nurses with any queries. APNA may also make contact to check how nurses have been implementing learnings from this program.

Cancellations and refunds

Event cancellations must be made in writing to APNA via email admin@apna.asn.au.

Refunds are not available within 7 days of the workshop.
