NiPHC Chronic Disease Management and Healthy Ageing Program | June 2024
This virtual workshop is the first stage in our 6-month intermediate level Chronic Disease Management and Healthy Ageing Program.
Tailored for nurses seeking to advance their skills in the focus areas of cardiovascular health, cancer control, injury prevention and control, mental health and diabetes mellitus. This course presents comprehensive strategies and is aimed at enhancing existing practices and expanding professional expertise. If chronic disease management is not currently in your existing practice, please view APNA Principles of Chronic Disease Management which is for nurses to develop and apply skills in chronic disease management, developing foundational knowledge and building practical strategies.
- To enhance understanding of the principles of chronic disease management and healthy ageing for five key chronic conditions.
- To incorporate and apply the concepts of chronic disease management and healthy ageing within nursing practice.
- To identify strengths and areas to grow in relation to chronic disease management and healthy ageing practice.
Dates of Workshop:
Tuesday 11th of June, time in each state below:
- Victoria: 12-2pm
- Tasmania: 12-2pm
- New South Wales: 12-2pm
- Queensland: 12-2pm
- South Australia: 11:30-1:30pm
- Northern territory: 11:30-1:30pm
- Western Australia (AWST): 10-12pm
One workshop per month after our induction, nurses will be guided through their role in empowering patients to manage chronic disease and healthy ageing. The key focus areas are:
- Cardiovascular health
- Cancer control
- Injury prevention and control
- Mental health
- Diabetes mellitus
Dates and times of each workshop will be announced one month prior to each session.
Upon registering for the workshop, nurses will complete a self-assessment (online survey) to develop of an understanding their current knowledge, skills and confidence and identify any areas for improvement. From this, we link nurses to key APNA online learning to complete.
APNA Online Learning
APNA provides online learning modules of approximately 12 CPD hours to nurses. We encourage nurses to use their self-assessment as a guide through their personalised learning journey. These are optional and nurses are not required or expected to complete all of the learning activities.
Community of Practice
A private messaging platform where program participants can discuss, engage and consult with their peers. APNA provides tips and resources to the group, as well as encouraging and facilitating discussion.
Meet your CDMHA Team
Cressida Bradley | Program Solutions Manager - Leadership, Education and Support
Cressida is the Program Manager for CDMHA and you'll see her in the workshops and online in the Community of Practice. She has a nursing background with experience in aged care, acute care and as a research nurse in gerontology. She's also previously worked in universities developing and delivering support and wellbeing programs for students. Before joining to the CDMHA team she worked in APNA's Building Nurse Capacity program helping nurses set up nurse clinics. She loves supporting primary health care nurses to develop their skills and grow their careers. She's currently studying a Masters of Public Health at Flinders Uni.
Alisha Komninos | Project Coordinator
Alisha will be your go-to individual for any issues and clarify your need, she will assisting with administration, and webinar hosting/ IT help ensuring you have all resources at arms reach!
