Introducing Cultiv-8

Program Format

  • Eight-week program​
  • Eight clinics per program
  • Two face-to-face sessions (weeks one and eight)
  • Six online sessions (weeks two to seven)
  • ​Nurse Support provided by the BNC team​​
  • Lead Nurse Accountability Check-in (three months post-program completion)

Program Structure

  • Week 1 - Three hours/in-person, Topic - 'Introduction' (workshop)
  • Week 2 -  One hour/online, Topic - 'Clear Plan' and 'Location and Facilities'
  • Week 3 - One hour/online, Topic - 'Staffing and HR' and 'Best Practice'
  • Week 4 - One hour/online, Topic - 'Patient Engagement' and 'Systems and Processes'
  • Week 5 - One hour/online , Topic - 'Funding'
  • Week 6 - One hour/online, Topic - 'Data Collection'
  • Week 7 - One hour/online, Topic - 'Evaluation'
  • Week 8 - Three hours/in-person, Topic - 'Close' (workshop)
  • Three months - Online Survey, Topic - Three month accountability check-in

 

Learning Objectives

At the completion of the eight-week program, participants will be able to:​

  • Explain the value of nurse clinic models.​
  • Apply APNA’s Nurse Clinic Building Blocks to plan a nurse clinic. ​
  • Identify funding options to support nurse clinic models of care.​
  • Determine a range of measures suitable to assess a nurse clinic.​
  • Access APNA’s suite of Nurse Clinic resources.​
  • Develop an implementation plan to establish a Nurse Clinic.
